Group Dental

History
Bollinger has been providing group dental expense insurance coverage to school boards, municipalities and commercial groups for more than 30 years. Our plan is fully insured by Monumental Life Insurance Company, which is currently rated A+ (Superior) for financial strength and operating performance by A.M. Best Co. (Ratings and Analysis 2007).

Plan Design
In our position as exclusive Managing General Underwriter for this program, we can offer a wide choice of benefit options to meet our customers’ needs.

Freedom of Choice
Individuals who are covered by a Bollinger Group Dental Plan may use any dental provider they choose. There are no penalties or restrictions associated with their choice of dentist.

Claim Payment
Claim payments are adjudicated by our own claims staff, and in most cases are processed within 24 hours.

Optional PPO
Bollinger has contracted with the NovaNet/Aetna Dental Administrators network of dental providers. Participating providers have agreed to accept discounted dental claim payments. Utilization of a network dentist is voluntary, however, insureds who do utilize a network provider can in most cases reduce their out-of-pocket costs. On a nationwide basis, the NovaNet network includes more than 73,000 participants. A listing of dental providers can be found at www.novanetppo.com.

Usual & Customary Payments
Plan benefit allowances are based upon information purchased from Medical Data Research, Inc. Their survey provides accurate and current information regarding dental charges and is specific to each procedure code and postal zip code.

The Bollinger Dental Program is available to groups of 10 or more employees located in the following states:
